No Physician-Patient Relationship
Use of this Site, including submission of contact forms, emails, or appointment requests, does not establish a physician-patient relationship. A physician-patient relationship is established only after a formal appointment has occurred and both parties agree to enter into a treatment relationship.
Medical Emergencies
This Site is not intended for medical emergencies. If you are experiencing a medical emergency, call 911 or seek immediate medical attention.
Appointments and Services
Appointments, health programs, and services offered through SlimWell Health are subject to availability and may be modified or discontinued at any time. Fees, policies, and services are subject to change without notice.
Cancellation and Refund Policy
A full refund will be provided for appointments, body composition assessments, or health programs canceled at least 48 hours before the scheduled appointment or program start date.
For individual services, including New Patient Visits, Follow-Up Visits, and InBody Body Composition Assessments, no refunds will be issued after the service has been provided.
For health programs, refunds requested after the program has started will be prorated based on the regular cash value of services already provided, including office visits, InBody assessments, and any other services received as part of the program. The value of services rendered will be deducted from the program fee, and the remaining balance, if any, will be refunded.
Appointments canceled with less than 24 hours' notice or missed without notice may be charged the full appointment fee or counted as a completed visit within a health program.
Intellectual Property
All content on this Site, including text, graphics, logos, images, and other materials, is the property of SlimWell Health unless otherwise indicated and may not be copied, reproduced, distributed, or used without written permission.
Third-Party Links
This Site may contain links to third-party websites. SlimWell Health is not responsible for the content, accuracy, privacy practices, or availability of external websites.
Limitation of Liability
